The Rules 

The following rules are designed to make the Striving for Excellence Awards 2017 nomination and entry process as clear and fair as we can.

  • Individuals and teams can not nominate themselves for an Award.
  • Nominations for the League of Friends Customer Care Award can be made by members of the public e.g. patients, visitors, and carers
  • Nominations for all other categories can be made by downloading the nomination form from the hospital intranet
  • The nomination must include examples of work within the last 12 months and clearly demonstrate how individuals or teams have gone the ‘extra mile’. Without examples we are unable to judge the nomination.
  • There is no limit to the number of nominations that can be made by an individual, team or organisation.
  • All nominations must be received by 5pm on 30 November. Nominations received after this date will not be considered.
  • Decisions made by the judging panels are final.
  • Nominators have the right to remain anonymous (their names are not disclosed to their nominee). Please ensure you make it clear when completing the form by ticking the appropriate box
  • The judging panel reserve the right to select an alternative category for your nominee to be considered in.
