The Staff Club Exercise Referral Scheme is designed to improve the health and wellbeing of people with health conditions that can be improve or controlled by taking part in regular physical activity. The scheme is delivered by staff qualified to level 3 in Exercise Referral Management.
People can be referred for membership by their GP, Consultant, Specialist nurse, Practice Nurse and other healthcare professional. (i.e. Physiotherapists, Occupational Therapists, Psychologists).
Many health conditions can be improved or controlled by taking part in regular physical activity including:
• Mental health i.e.; mild to moderate depression including post natal, negative mood and low self esteem
• Coronary Heart Disease i.e.; prevention and Cardiac Phase 4 rehab
• Muscular-Skeletal problems i.e.; back pain
• Increased mobility and flexibility
• Respiratory conditions
• Diabetes Type 1 & 2
• Osteo and Rheumatoid Arthritis
• Pre and Post Natal Exercise
• Preparation for surgery
The Referral Member will have a programme developed and tailored to their needs by one of the 7 qualified Fitness Referral Officers following an appointment. This program is specific to the activities they enjoy, activities determined by their health condition, prescribed drugs and preferences and includes monitoring of their condition and fitness outputs. The Referral Officer supports each referral member through their prescribed fitness program over 12 weeks.
After completion of the scheme and with the support of the Referral Officer a future action plan is developed for the member to continue to be physically active.
The Staff Club offer a Graduate Referral membership for those patients who have a medical condition which requires on going activity to maintain or improve health.
