Patient Outcomes

What are they?

They are simple questionnaires that we will ask you to fill in at different times during your treatment.  They will ask you about your current symptoms and how they make you feel. We currently have four patient reported outcome questionnaires:

If you have been sent a questionnaire but misplaced it, a copy can be printed by clicking on the relevant questionnaire above.

Why do we use them?

We collect this information from you so that we can:

  • Measure your health when you first come to us.
  • Monitor your health as it changes through time.
  • Compare your response to treatment with other patients.
  • Make better treatment decisions about you.

How else is this information used?

The information we collect is also used to monitor the clinical performance of the team as a whole, as well as individuals within it.  Outcome data is available to the team through an electronic urology dashboard (a tool for viewing how we are doing) which is automatically updated as new information is entered.  This is then discussed at a monthly team meeting along with other performance information, such as waiting times and compliments and complaints.

Improving services for urology patients

As well as using the urology dashboard to monitor clinical performance, we also use it to identify areas where the service can be improved.  A recent change in practice as a result of the information on the dashboard has been the development of a telephone follow-up clinic which allows patients to be assessed by a urology nurse specialist at home instead of coming to the hospital.
