Question Number 1: Please provide a full organisational structure chart for Salisbury NHS Foundation Trust inclusive of all ‘Clinical Directors’. Can I please request their full job titles and the names of the individuals who hold these positions.

Question Number 2: Please can you confirm the named Trust ‘NHS Pensions Officer’, and appropriate contact details.

Answer To Question 2: The Trust's NHS Pensions Scheme Officer can be contacted by calling the main hospital telephone number 01722 336262 and they will be able to transfer you through to the appropriate person. Trust policy prohibits the publication of staff details unless they are on Agenda for Change band 8a and above or equivalent pay.
