New Discharge Grant Scheme available to support discharge from hospital

A new scheme to support patients to get home as soon as possible will be launched at Salisbury District Hospital the week beginning 17th January 2022. Salisbury District Hospital is the first site to trial the scheme with Great Western Hospitals NHS Foundation Trust and Royal United Hospitals Bath NHS Foundation Trust following.

The Discharge Grant Scheme will provide a grant to support individuals with small packages of care and assistance that may help them return home.  For example, this could include additional help with cleaning or someone to help with cooking and shopping.  The support of informal care to the patient could be provided by a family member or from another individual or company, dependent on the need. The funding will support the individual for four weeks post discharge.

Patients that are clinically ready for discharge from hospital will be assessed to ensure they meet specific criteria and will then be able to receive the grant. 

The local NHS Clinical Commissioning Group is working with CHS Healthcare to deliver the scheme.
