Question Number 1: Under the Freedom of Information Act 2000, I would like to request the following information: Do you have a system to track colorectal patients on care protocols that allows remote monitoring or PIFU or is this currently a manual solution/recorded using a spreadsheet?

Answer To Question 1: Yes My Medical Record for those patients on self supported management with surveillance.

Question Number 2: If you have a system which system do you use and does it have a portal that the patients can access?

Answer To Question 2: Yes patients have the choice of IT user or non It user. IT user gives access to a portal.

Question Number 3: If you have a system when is this contract due for renewal?

Answer To Question 3: March 2026

Question Number 4: What is the annual cost of supporting this system?

Answer To Question 4: £27,879 (this is for 3 sites Prostate, Breast and Colorectal)
