Question Number 1: Could you please provide details of the providers you commission to deliver POTS services? We are currently aware of the following providers: Salisbury District Hospital.

Answer To Question 1: The Trust are no longer seeing patients with a diagnosis of POTS as it is a diagnosis that can be made in primary care and managed in primary care. It is not a cardiac diagnosis but one of dysautonomia.

Question Number 2: Do any of your providers use the Electronic Referral Service (ERS)? If so, could you please provide the details?

Answer To Question 2: The Trust do not see people out of area therefore ERS referrals are rejected.

Question Number 3: Are there any providers that accept Out of Area (OOA) referrals? If so, could you please provide specifics?

Answer To Question 3: The Trust do not see people out of area.

Question Number 4: What are the exclusion criteria that your providers use to reject patients?
