Question Number 1:
I am writing to make a request under the Freedom of Information Act 2000.

Please provide the following information in relation to patients treated by your Trust:

The total number of incidents reported involving missing, lost, or damaged dentures belonging to patients while under the care of your Trust.

Please provide this data for each of the last five financial years (or the maximum period available if five years is not held) ie 2020 to and including 20253.

If available, please break the data down by hospital site (where your Trust operates more than one hospital)

For clarity, this request includes (but is not limited to) incidents recorded via:

- Incident reporting systems eg Datix
- Complaints eg Patient Advice and Liaison Service
- Claims or risk management records
- The finance department to include reimbursement claims or financial costs associated with lost or damaged dentures
 
Answer To Question 1:
None reported via the incidents module on Datix.
There are no documented Risks involving dentures.
No complaints have been raised regarding lost or damaged dentures however over the last 5 years we have had 36 notifications from patients regarding lost dentures.
The number of compensation claims paid and their value are attached.

- The maintenance of oral hygiene for patients

- The management, labelling, storage, or safekeeping of patients’ dentures or other removable dental appliances

- The reporting and handling of incidents involving lost, missing, or damaged patient property (where dentures are included or referenced)
