Question Number 1:
I would like to request the following information for the period 2021-2022 or 2022-2023 whichever data is available easily: in essence we are trying to gauge how many labs have to re-label blood tubes, and the size of the problem. If you have precise data that would be preferable, but if it is easier to give approximate data, that is fine also

Answer To Question 1:
In 22-23 we rejected approximately 6100 tubes 563 were unlabelled, but tubes can need relabelling for many reasons.
