| Question Number 1: This request forms part of a doctoral research project exploring approaches to reducing wrong tooth extraction and improving patient safety within dentistry. Please could you provide the following information relating to the Oral & Maxillofacial Surgery, Oral Surgery, Hospital Dental Services or similar departments at SALISBURY NHS FOUNDATION TRUST. Part A – Clinical activity and patient safety data For each of the last three complete years, please provide: 1. The total number of procedures coded under the following OPCS-4 categories: • F09 – Surgical removal of tooth • F10 – Simple extraction of tooth 2. The number of recorded incidents relating to: • wrong tooth extraction • wrong-site dental extraction procedures • relevant Never Events involving dental extraction procedures Where possible, please provide annual totals only.
